Issue Adding Task on Arize Platform: Save Button Unresponsive

Hi all, I'm trying to add a task on the Arize Platform, but the platform seems unresponsive to the click on the "Save Evals" button, and the Create button is grayed out so I'm unable to save the new Task. All the fields are entered correctly, but I might be missing something. I would appreciate any help. (Tried on both Chrome and Safari but had the same issue on both Browsers)
